The canvas is by Leigh Designs from her Wicked series and the stitch guide by Amy Bunger.  You can pick up the guide and thread kit from Amy if you have the canvas already.  Or you can buy the canvas and guide from Amy and use your stash.   Now that Amy's closed her shop, you'll have to order the canvas and threads from your favorite shop but you can still get the guide from Amy.
